Position located at Heidelberg Repatriation Hospital, working with clients at our partner Aged Care Facility and in the community.Full time or part time fixed term 12 month opportunityExciting opportunity to trial a Nurse Practitioner led model for the Transition Care Program Austin Health is the major provider of tertiary health services, health professional education and research across three campuses in the north-east of Melbourne and statewide. About the Transition and Restorative Care ProgramsThe Transition and Restorative Care Programs sit within Aged Care Community Services as part of the Continuing Care Division at Austin Health. The programs are committed to providing high quality safe care for patients while helping them achieve their goals. The programs are provided in both residential and community settings. About the roleNurse Practitioners (NP) are supported by Austin Health to work in clinical specialties where there is evidence that the advanced clinical skills of a Nurse Practitioner would enhance patient outcomes within an efficient service model. As the Nurse Practitioner within the Transition Care Program (TCP) you will be responsible, in conjunction with the Geriatricians, in providing comprehensive medical assessments to clients admitted in the Residential or Community Programs. You will work with the multidisciplinary team to ensure that best practice care is provided to clients on the TCP program, supporting avoidable hospital presentations and facilitating discharge planning. Additionally, you will contribute to the department in other ways such as; education of staff, families and the community, research, practice and professional development, specialty and role promotion, quality improvement and clinical and professional leadership. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
