What you'll be doing Bear Cottage Children's hospice is part of Sydney Children's Hospitals Network and is located in Manly. Bear Cottage provides respite, symptom management and end of life care for newborns, children, adolescents/young adults with life limiting conditions. The Bear Cottage Pharmacist will support the delivery of a high-quality pharmacy service to enhance the clinical care of patients who are admitted to Bear Cottage for respite, symptom management or end of life. This includes a comprehensive medication management service that provides a safe and efficient medication supply for patients. The role will offer clinical guidance to specialist palliative care clinicians when required including advice on medication infusions, device and equipment usage and medication stability. The Bear Cottage Pharmacist position will work collaboratively with the SCHN Palliative Care Pharmacist role to develop an integrated Palliative Care pharmacy model for patients across the hospital, community and hospice setting. This Bear Cottage Pharmacist will be involved in the provision of training programs, research and quality improvement activities to improve paediatric pharmacy service capability in palliative care across NSW. The role will professionally report to the SCHN Director of Pharmacy , and operationally to the SCHN Palliative Care and Hospice Manager. About You We’re looking for someone who: Registered as a Pharmacist with AHPRA with no restrictions on practice. Substantial clinical pharmacy experience demonstrating strong clinical reasoning skills and medication knowledge, with a preference for experience in palliative care and/or paediatric cohorts. Demonstrated experience in problem-solving, quality improvement, and development and implementation of innovative solutions. Operational knowledge of the NSW Health medication workflows with the ability to adapt to provide high-quality medication services in a non-hospital, paediatric hospice setting.
