About this opportunity As a Care Manager, you will play a pivotal role in ensuring the highest quality of care for both potential and existing clients. You will lead and coordinate all activities related to client care. Key Responsibilities Build trusting relationships with prospective and existing clients, their families, and representatives. Conduct regular quality assurance visits (in person or via phone) to monitor care, address changing needs, and recommend additional services as required. Collaborate with clients, families, Caregivers, and other stakeholders to ensure care plans remain aligned with evolving needs and preferences. Maintain strong local knowledge and networks by staying informed about community services and engaging with health providers and referral sources to promote available support options. About you Tertiary qualification in a health-related field is desirable (e.g. social work, allied health) with at least 1 year of experience working with Home Care Packages and seniors in aged care. Strong organisational skills with the ability to manage time effectively, multitask, and adapt to shifting priorities under pressure. Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office.
