Nurse Practitioner – Primary Care (Core Role) Location: San Antonio, TX Pay Rate: $70.38/hour Schedule: Core clinic hours, including every other weekend and potential holiday rotation Employment Type: Full-Time, Patient-Facing Start Type: Individual Starts Position Summary We are seeking an experienced and dedicated Nurse Practitioner to join our primary care team in San Antonio, TX. The Nurse Practitioner will collaborate with a clinical care team to deliver high-quality, evidence-based care to a panel of patients. This position involves performing comprehensive assessments, diagnosing and managing acute and chronic conditions, providing health counseling, and coordinating care to improve patient outcomes. The role requires strong clinical judgment, excellent communication skills, and the ability to work both independently and as part of an interdisciplinary team. Key Responsibilities Provide primary and family care services, including accurate assessment, diagnosis, treatment, and management of health problems. Deliver evidence-based care and promote positive health outcomes for patients. Conduct patient exams and diagnostic testing to assist in treatment and care planning. Educate patients on preventive health practices, chronic condition management, and healthy lifestyle choices. Coordinate care with other healthcare professionals to ensure a comprehensive plan for each patient. Complete administrative duties such as opening/closing the clinic, managing inventory, verifying insurance, collecting payments, and maintaining a clean, compliant environment. Encourage patient engagement in health screenings, behavioral health, and wellness initiatives. Participate in clinical quality improvement efforts and adhere to established policies, procedures, and regulatory standards. Required Qualifications Master’s Degree from an accredited Family Nurse Practitioner program. Current National Board Certification and active state license to practice as an Advanced Practice Nurse in Texas. DEA registration required (or must be obtained upon offer acceptance). BLS certification required (must be provided upon offer acceptance). Minimum 1 year of recent experience in a primary care or family medicine setting preferred. Proficient in using electronic health record (EHR) systems and other healthcare technology. Strong interpersonal, written, and verbal communication skills. Highly organized with the ability to prioritize tasks and manage time effectively. Demonstrated ability to work independently and as part of a team. Willingness to obtain multi-state licensure, if applicable. Preferred Qualifications Experience managing chronic conditions and implementing preventive care programs. Knowledge of quality measurement standards in chronic disease management. Demonstrated adaptability, critical thinking, and problem-solving ability. Commitment to patient advocacy and continuous quality improvement. Work Schedule Core clinic hours, including every other weekend and potential holiday rotation. In-person, patient-facing role. Training schedule may vary based on start date and program needs. Education Master’s Degree in Nursing (MSN) with specialization as a Family Nurse Practitioner (FNP). Current, unrestricted license to practice as an Advanced Practice Registered Nurse (APRN) in the state of Texas.
