Position Overview As a Medical Assistant (MA), Case Manager, Social Work, RN, LPN, WA Health Home Care Coordinator, with Bridging Care, you will play a vital role in supporting patients with complex health needs. This role is primarily community-based, with most of your time spent conducting in-home visits, health screenings, and care coordination activities. You’ll collaborate with physicians, care coordinators, and community partners to ensure patients receive the support they need to thrive. Position Overview As a Medical Assistant (MA) with Bridging Care, you will play a vital role in supporting patients with complex health needs. This role is primarily community-based, with most of your time spent conducting in-home visits, health screenings, and care coordination activities. You’ll collaborate with physicians, care coordinators, and community partners to ensure patients receive the support they need to thrive. Key Responsibilities Conduct in-home and community-based visits with patients. Assist with health screenings, vitals, and documentation in the electronic health record (EHR). Support care coordination by helping patients navigate appointments, community resources, and follow-up care. Educate patients on preventive care, chronic condition management, and medication adherence. Partner with providers and care teams to implement individualized care plans. Promote patient engagement, wellness, and self-management skills. Complete accurate documentation and maintain confidentiality of patient records. Qualifications Active Medical Assistant Certification (CMA, RMA, or equivalent) in Washington State. Valid WA Driver’s License and access to a reliable personal vehicle. Preferred Current CPR and First Aid Certification . Strong communication and interpersonal skills; ability to work sensitively with diverse populations. Tech-savvy with proficiency in MS Office and electronic documentation systems (Teams/Excel/Word/PowerPoint) Preferred Bilingual or bicultural applicants are strongly encouraged to apply. Requirements Must reside in one of the following counties : King, South King, Pierce, Snohomish, or Kitsap. Willingness to travel locally (up to 90% field work - 4-5 Face-to-Face visits per day ; mileage reimbursed). Ability to work Monday – Friday, 9:00 AM – 5:30 PM Reliable internet for remote documentation. Compensation & Benefits Pay: $27.00 – $30.00 per hour (Full-time, 40 hours per week) Perks include: 100% Employer-paid medical, dental, and vision coverage.
