Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: 5 years of RN Case Management experience License: Valid RN License Certifications: BLS Must-Have: - CA RN License - 5 Years of RN Case Management experience Description: Case Manager RN needed for day shifts. This position is responsible for care coordination and care transitions planning throughout the acute care patient experience. The Case Manager collaborates with physicians, utilization managers, medical social workers, and bedside RNs to ensure the timely progression and transition of patients to appropriate levels of care. The goal is to prevent unnecessary admissions or readmissions for patients. This role encompasses effective resource coordination and communication across the continuum of care. Responsibilities include initial and continued patient assessment. The Case Manager reviews physician admission care plans and gathers additional medical, psychosocial, and financial information from patients and families. They determine risk levels for readmission and conduct screenings for ancillary supportive services. The Case Manager leads the healthcare team in developing comprehensive, cost-effective care coordination plans that meet patient needs. Identifying and referring quality and risk management concerns for patient safety is essential to this role. Utilization management is a key responsibility, ensuring that chart documentation supports the level of care determination and assignment. The Case Manager works closely with attending physicians to expedite transition planning for patients no longer requiring acute care. They collaborate with financial counselors regarding inpatient stay denials and participate in patient rounds with interdisciplinary teams. Monitoring length of stay and identifying outliers requiring additional resources is required. The Case Manager formulates transition plans by reviewing care options and collaborating with patients, families, and healthcare teams. They proactively identify barriers to care progression and work to resolve them timely. Assessing the need for follow-up appointments and ensuring necessary paperwork for post-acute transfers is part of the responsibilities. The Case Manager identifies high utilizers and refers them to community resources as necessary. Effective communication with care management colleagues and team members is essential for safe transitions and operations. The Case Manager actively contributes to the creation of cost-effective practices to ensure optimal patient/provider experiences and outcomes. Maintaining productive relationships with healthcare teams and community agencies is crucial. The Case Manager must manage all communications effectively and participate actively in department operations. Providing and accepting constructive feedback is an integral part of teamwork and professional growth. This role requires a high level of interpersonal and communication skills to promote customer service effectively.
