Utilization management duties include reviewing medical records to confirm level of care. Transition planning for patients will be expedited as necessary. Collaboration with attending physicians is essential for documentation and support of care levels. Barriers to care progression and transitions will be identified and resolved. The position requires knowledge of regulations and standards related to patient care. Care transitions will be planned in conjunction with patients, families, and healthcare teams. Timely referrals and communication with community resources are necessary. The Case Manager will actively participate in department operations and process improvements.
