This role involves providing quality in-home care under various funding sources, liaising with clients, families, staff, and stakeholders, and managing care plans and services to ensure client needs are met with a flexible and collaborative approach. Responsibilities also include managing sales inquiries, developing referral sources, and maintaining accurate records. Responsibilities Qualifications Building strong connections with clients and families Assigning staff based on qualifications, availability, and client preferences Supporting clients with disabilities, monitoring progress, and referring to additional services Assessing needs and resources for health and welfare Responding promptly to communications Maintaining incident and feedback records Prioritizing work in a fast-paced environment Supporting clients in achieving their goals Conducting assessments and care planning Supervising and managing care staff Promoting services to medical professionals and referral sources Reporting to head office Ability to work in a fast-paced environment.
