TITLE: Housing Nurse Care Coordinator PROGRAM: Housing Services JOB SUMMARY: The focus will be on supporting our housing program (Winona) and psychiatric department needs (Red Wing/Wabasha) for clients with SPMI/SMI. JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS: Assist clients with medication management and administration in onsite medication rooms. Conduct general health assessments and provide dietary or other physical health advice as necessary and appropriate. Participate with other health care providers in the development and modification of a plan of care. Follow-up with clients on medical appointments and support collaboration of provider services as appropriate. Establish collaborative, constructive working relationships with community service providers, housing employees and clients. Provide consumer education on the basic principles of physical and mental health, as well as interpersonal and social skills, as assigned. Attend appropriate conferences and in-service training sessions as necessary to meet licensure and program requirements. Participate in clinical supervision as directed. Participate in Housing Services weekend emergency on-call rotation as assigned. PHYSICAL REQUIREMENTS FOR POSITION: Must be able to move in a manner conducive to the execution of daily activities. While performing the duties of this job, the employee must communicate with others and exchange information. The employee regularly operates equipment (listed below) on a daily basis. Occasional bending and lifting of office materials may be required. NON-ESSENTIAL FUNCTIONS: Perform other duties as assigned by the Program Director. EQUIPMENT USED: Including but not limited to: Telephone/cell phone, vehicle, computer, scanner, blood pressure cuff, thermometer, stethoscope, scale. JOB QUALIFICATIONS AND REQUIREMENTS: Registered Nurse (RN) OR Licensed Practical Nurse (LPN) able to practice in Minnesota. Must meet minimum qualifications as a Mental Health Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN Statutes Able to work with clients with severe and/or persistent mental illness, previous experience providing services to mentally ill clients preferred. Exceptional organization and communication skills (oral and written). Decisive and accurate decision-making ability is critical. Team player, with strong ability to work with peers, clinical and administrative supervisor, community partners, and other clinical staff. Possess a high level of human relation skills and the ability to work with and react appropriately to a variety of people in difficult or challenging situations. Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations. Must be able to maintain confidentiality. Must possess a valid drivers license and a willingness to travel as needed to client or organization locations throughout SE MN. Must hold a current certificate in CPR/AED/First Aid or be willing to obtain. SUPERVISED BY: Program Director SUPERVISES: None POSITION DESIGNATION: Non-exempt, Full Time or Part Time This job description is subject to change at any time. BENEFIT SUMMARY: Paid Leave Time starts accruing at 5.08 hours per pay check (pro-rated for employees under Full-Time). Up to 10 days of paid jury duty leave. Up to $2,000 Tuition investment and up to $1,500 continuing education funds available.
