Job Description Job Description LEAD CASE MANAGER Full Time $1500 sign on bonus$$ Overview The lead case manager will determine resources needed to meet the individualized needs of the participant and will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. This position functions as a collaborative team partner among RHD associates and includes daily support and mentorship to a case management team, reporting directly to the Case Manager Supervisor to address participant’s needs. A commitment to RHD values should be demonstrated as job duties are performed. Program Summary: RHD Montgomery County Homeless Services and Genny’s Place offers a safe place to stay and assistance for individuals experiencing homelessness, helping them transition towards independent living. The program is a partnership between Your Way Home, Montgomery County’s Office of Behavioral Health, and Resources for Human Development. This is a new site located in Lansdale, PA, and the program specifically caters to single adults without housing. Clients have access to dedicated caseworkers who help them find permanent housing and apply for necessary benefits. Additionally, we provide practical support such as laundry facilities, transportation, meals, clothing, and essential life skills training. Major Responsibilities Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective. Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant. Provide community-based services to program participant focusing on education, connection with resources, community interaction and achievement of recovery plan goals. Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities. Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources. Serves as field support to the case management team to include, but not limited to, guidance related to service delivery and documentation, maintaining compliance with regulatory standards, appropriately managing caseloads, and coordinating on-call scheduling. Effectively self-manages and collaborates with all levels of staff and participants with varying needs ensuring timely and efficient utilization of resources, ensuring adherence to organization and regulatory requirements Provides case management expertise and mentorship to new case managers and/or case managers challenged with performing tasks to include but not limited to assessments, treatment planning, supportive counseling, linking to community resources, monitoring progress, collaboration with other providers, and care coordination. Manages high-priority referrals from program funders and stakeholders Assists Case Managers with necessary outreach and engagement activities to include participants and their families, community providers, landlords and medical personnel Ensures adherence to oversight entities’ regulations regarding paperwork and service delivery Act as a representative for the agency at stakeholder meetings. Other duties as assigned Job Qualifications: Bachelor’s Degree in Human Services or related field. Two to four years Case Management experience preferred Three years of experience providing support to individuals in behavioral health, addiction recovery or development disabilities services required. Well-organized and detail orientated Excellent Customer service skills (both internal and external) Strong interpersonal and communication skills Leadership and management skills Resident relations and retention Knowledge and understanding of state and federal regulations regarding group home/ residential care Behavioral issue management and crisis intervention skills Compliance with regulatory standards Proficiency in direct supervision, developmental disabilities care and medication administration Life saving measures (CPR) and first aid certifications Budget management Previous supervisor/management experience in group home/residential setting Medical terminology knowledge Demonstrates a strong working knowledge of the appropriate regulations, Apis values and program policies and procedures. Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job. Valid non-provisional drivers’ license and personal vehicle with state minimum liability insurance coverage. Subject to employment screenings, including FBI clearance, if applicable.
