Job Title: Clinical Nurse Profession: RN Specialty: Stepdown Duration: 13 weeks Shift: 7AM - 7:30PM EOW Hours per Shift: 12 Experience: Minimum of 2 years RN experience, 2 years of cardiology, tele, VAD experience License: NYS RN License Certifications: BLS Must-Have: Bachelor of Science with a major in nursing preferred; Associate degree in nursing considered with relevant experience or active matriculation in a BSN Program Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and applies the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care, while delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention, while anticipating needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates effective allocation of material resources. Role models the tenets of Relationship Centered Care through facilitation of various patient-centered actions. Considers the patient's values, preferences, and cultural diversity in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patients and families with concerns and escalates to appropriate management as needed. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions. Practices safe and efficient patient-centered care while identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional evidence and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and evidence-based practice activities. Contributes to quality and safety practices and compliance measures. Implements principles of high reliability to provide a safe environment for patients. Escalates potential safety hazards or gaps in best practice following established protocols. Utilizes institutional information technology by accurately documenting nursing practice to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with management to ensure appropriate utilization of supplies and inventory for patient care. Escalates resource deficits to management when necessary. Demonstrates patient-focused and cost-effective approaches to patient care. Documents thoroughly in compliance with regulatory and legal requirements and standards of care. Facilitates patient throughput on shift basis. Contributes to an encouraging environment for clinical colleagues, patients, and their families. Advances clinical competence in nursing practice. Projects a professional image to colleagues while demonstrating effective communication styles. Incorporates ethical principles into decision making for patients and families. Encourages a spirit of scholarship, inquiry, and lifelong learning for self and others. Acquires knowledge and skills relevant to the role, patient population, and clinical specialty. Participates in shared decision-making through specialty practice councils and unit initiatives.
