Job Description Job Description Job Title : LPN Patient care Specialist (Bilingual Spanish Required) Location : Hempstead, NY 115 Employment Type : Part-Time Or Full-Time Salary Range : $42 - $45 per hour Key Responsibilities : Initiate follow-up calls to patients shortly after their clinical visits to assess care and gather feedback. Ask standardized questions to evaluate if patient concerns were addressed and gather suggestions for improving their care experience. Confirm current medication use, identify any newly prescribed medications, and assess adherence and challenges like side effects. Verify that follow-up appointments are scheduled and address barriers such as medication access, transportation, educational gaps, food insecurity, and emotional concerns. Conduct basic health assessments and symptom checks (e.g., pain, shortness of breath, dizziness, nausea). Document all patient interactions thoroughly in the medical record. Perform depression screenings when appropriate and escalate concerns to clinical leadership. Ensure patients understand their care instructions, including medication regimens. Collaborate with the care team to address any unresolved patient needs or barriers. Support patient engagement and optimize their experience with healthcare services. Qualifications : 3-5 years of experience as an LPN or similar role. Bilingual fluency in Spanish is mandatory . Strong communication, empathetic listening, and documentation skills. Ability to work independently and as part of a multidisciplinary team. Experience in patient follow-up, care coordination, or outpatient settings preferred. Benefits (Full-Time after 90 days): 50% company-paid health insurance (select plans). 50% paid for Jewish Holidays. 100% paid for six major federal holidays. PTO accrual starting on day one; available after 90 days (15 days for LPNs, with different tiers for other roles). No 401(k) offered. Opportunity to work as a 1099 contractor if preferred.
