Part-Time Care Navigator | £12.25 - £12.40/hr | Immediate Start A friendly and patient-focused GP practice is seeking an enthusiastic and motivated Part-Time Care Navigator to join their expanding team in Poole. They are looking for someone with excellent customer service skills, strong IT knowledge, and a passion for helping patients access the right care at the right time. Role Details: Hours: 34 hours per week (1 slot available) Salary: £12.25 - £12.40/hr Contract Type: Permanent, Part-Time Location: Poole Key Responsibilities: Triaging and signposting patients to the appropriate services Identifying serious cases and escalating to the clinical team Booking and managing appointments promptly and accurately Building strong working relationships with clinical and admin staff Engaging with patients via phone and messaging with empathy Supporting care planning and reducing unnecessary hospital admissions About You: GCSEs in Maths and English (or equivalent) Minimum 1 year’s experience in a customer service role Confident using MS Word, Excel, and Outlook Previous NHS experience is a plus but not essential Able to work with empathy, accuracy, and initiative This is a fantastic opportunity to join a welcoming, patient-centred practice where your contribution will be truly valued.
