This part-time opportunity is ideal for a dedicated clinician who values flexibility, continuity of care, and a supportive team environment. There is also an optional opportunity for rounding in a skilled nursing facility (SNF) one day per week. Key Responsibilities: Independently manage a panel of adult patients under a collaborative agreement with a supervising physicianConduct physical exams, manage chronic and acute conditions, and provide preventive careDevelop and implement individualized treatment plans based on clinical judgment and best practicesMaintain accurate documentation in the electronic medical record systemCollaborate with physicians, nurses, and staff to provide coordinated, high-quality careOptional: Provide skilled nursing facility (SNF) rounding one day per week Schedule: Part-time: 1–2 days per week (Thursdays and Fridays), No nights, weekends, or holiday call, Optional SNF rounding available.
