Job Duties This position requires an experienced advanced practitioner, prior experience working in a skilled nursing facility (SNF) is helpful but not required. The SNF advanced practitioner (AP) provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting. Overview: Hours: Monday – Friday 8 a – 4:30p Responsibilities include: Communicate with SNF staff and review patient reports Treat acute needs, admissions, discharges, etc. Manage complex and non-complex patients Average 12 patients a day Length of time and number of patients as per need Locations may include, but are not limited to: Mountain Top Rehabilitation & Healthcare Center, Mountain Top Birchwood Rehabilitation and Healthcare Center, Nanticoke Lakewood Rehabilitation & Healthcare Center, Nanticoke The Meadows, Dallas Embassy of Wyoming Valley, Wilkes Barre, Embassy of Scranton, Scranton Oak Ridge Rehabilitation and Healthcare Center, Scranton Position Details Family Nurse Practitioner, Adult – Geriatric Nurse Practitioner or Physician Assistant with advanced practice experience in a skilled nursing facility or long-term care facility is preferred.
