SUBSTANCE MISUSE PRACTITIONER, NOTTINGHAM, FIXED TERM TILL MARCH '26, 37HRS, BAND 4 £26,530 - £29,114 , FTE The Rough Sleepers Drug & Alcohol Treatment Team (RSDATT) has a vacancy for the post of a Substance Misuse Practitioner. The service provides drug and alcohol treatment to rough sleepers or those at a risk of homelessness experiencing severe and multiple disadvantages. Treatment focuses on addressing health inequalities through assertive outreach and in-reach provision using an integrated model of care with effective interdisciplinary and interagency working. The successful candidate will: Conduct comprehensive assessments. Deliver harm reduction-based interventions and structured treatment. Prioritise access to opioid substitute treatment. Coordinate care for individuals with complex health and social care needs through assertive engagement, SMART care plans and involvement of specialist teams/agencies. Assist rough sleepers in accessing emergency or temporary accommodation. Provide advice on welfare, housing and tenancy rights. Facilitate assessments of care needs. The post holder will have the ability to develop and sustain therapeutic relationships based on trust, provide person-centered care, and deliver psycho-social support underpinned by Trauma Informed Care in a Psychologically Informed Environment. The role entails acting as an advocate; promoting autonomy, choice and independence in addition to establishing and reconnecting service users with positive social support networks and local community support services. A core aspect of the role is carrying out risk assessments and implementing effective risk management plans including for safeguarding concerns and crisis management. The practitioner should have an awareness of legislation, best practice guidelines and local procedures in particular areas relating to substance misuse, mental health, adult and child safeguarding, homelessness and community care. The post holder will be expected to deliver treatment based on the principles of equality, justice and culturally sensitive practice. It is essential that high standards of professionalism are maintained with accurate case management records and performance data. The successful candidate will be supported through training, supervision and shared expertise across the interdisciplinary team. There is an expectation that within reason, the candidate will be expected to carry out other duties to maintain service delivery, this includes covering the Rota on Saturdays. Why Join Us?
