Social Worker - South Locality Team (Herefordshire Council) Make a positive impact on the lives of adults in your community. Herefordshire Council is seeking a highly motivated and experienced Social Worker to join our South Locality Team. About the Role: Conduct comprehensive needs assessments of adults, identifying support requirements and developing person-centered care plans. Work collaboratively with individuals, families, carers, and other agencies to coordinate services and ensure holistic support. Advocate for the rights and needs of adults within your caseload, promoting their independence and well-being. Manage a diverse caseload, balancing competing priorities and ensuring timely interventions. Contribute to the ongoing development and improvement of services within the South Locality. Requirements: Registered with Social Care England. Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience working within a locality team is desirable. Excellent assessment, communication, and case management skills. Strong understanding of relevant legislation and policies relating to adult social care.
