The Per Diem Nurse Practitioner (NP) will conduct home health risk assessments within Las Animas County, CO. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Trinidad, COand the surrounding Las Animas County, CO.Las Animas County, COincludes locations like Trinidad, Aguilar, and Jansen.Candidates could be scheduled in any one of these locations during an individual shift.
