Clinical Aged Care Assessor - South West Sydney

Location - South West Sydney ( Travel required around lower South West Sydney areas such as, Mossvale, Bowral, Goulburn and Warragamba) Permanent part time - 3 days per week; flexible days – Monday to Friday; no shift work! ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. We have an exciting opportunity for a Clinical Aged Care Assessor to join the team. What will your role look like? You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care ACT Day to day this includes: Collaborating with clients, families and multidisciplinary teams as required Working collaboratively with colleagues to promote a supportive and positive team culture Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services Reviewing support plans to meet client’s changing support needs Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
