Job Summary Under the supervision of the pharmacy manager and/or pharmacy supervisor, performs all duties necessary to provide for the efficient daily clinical operation of the pharmacy, providing quality products and services to patients as well as act as a drug information resource for internal stakeholders. Duties And Responsibilities Ensures compliance with federal, provincial, and local laws and regulations, policies and procedures and accrediting body standards and practices pharmacy to their full professional scope Obtains, interprets, and evaluates all physician's medication orders to assure therapeutic appropriateness, cost effectiveness, accuracy, and completeness. Provide technical and clinical information and communicates results of monitoring activities to patients, nurses and physicians as required regarding the contents, utilization, therapeutic actions, possible adverse reactions, or interactions of drugs. Participates with nurses and physicians in all clinical activities as required. Works cohesively with and delegates appropriately to other team members including but not limited to pharmacy personnel, patient support programs, scheduling team and nursing team Communicates with internal and stakeholders as appropriate to provide seamless collaborative patient care Monitors patients on treatment by performing routine follow ups. Documents and collaborates with other stakeholders as required based on the result of the follow up. Initiates, reviews, and updates all Bayshore required documentation per Policy and Procedure (P&P). Follows established procedures for drug and supply procurement and maintains appropriate and accurate inventory levels and preferred product selection of drugs, solutions, and supplies including narcotics and controlled substances. Ensures the quality of medications are maintained throughout the pharmacy workflow up to and including patient receipt Report adverse events per Policies and Procedures Accurately maintain all legally required records. Assist in training and orientation of new employees. Performs and documents compounding checks and double checks per P&P. Prepares and/or checks infusion supply orders for patients. Prepares and/or checks equipment programming e.g., infusion pumps Prepares and/or checks compounding sheets for compounding. Adheres to stability and compatibility guidelines and quality control procedures. Performs accurate computer order-entry, labeling and dispensing in accordance with College of Pharmacist regulations. Participates in Performance Improvement activities, educational programs, and departmental/branch meetings. Understand full operation of the pharmacy which may require performing other duties as assigned Qualifications To perform this job successfully, as an individual, one must be able to perform each essential duty satisfactorily. Education and Training Bachelor's Degree (BSc) in Pharmacy or a PharmD, from an accredited college or university Licensed and in good standing with Provincial College of Pharmacists to be able to provide direct patient care Injection training (asset) CPR (asset) Other Skills And Abilities Strong attention to detail within a fast-paced environment Clinically focused Experience with Kroll computer software Strong communication skills via phone/email and in person Ability to multitask and work well in a team environment Motivated, self-disciplined, and able to work independently Computer ability (word processing, database, and spreadsheet programs, etc.) Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to define problems, collect data, establish facts, and draw valid conclusions. Interpret a variety of instructions furnished in written, oral diagram or schedule form. Physical Demands The employee is regularly required to talk or listen. The employee is regularly required to use hands and fingers to reach items, carry out dispensing activities and use a computer and other equipment The employee may be required to lift 25-50lbs The employee is frequently required to stand, walk, and sit.
