About the Role As an NDIS Support Coordinator / Psychosocial Recovery Coach , you will play a vital role in supporting participants to: Navigate and implement their NDIS Plan within the NDIS framework and available community services. Coordinate multi-disciplinary teams and services to meet participant needs across funded, mainstream, and informal supports. Identify appropriate services, engage providers, and guide participants toward achieving their goals. Address points of crisis, barriers, and risks holistically, developing sustainable solutions. Build and maintain strong partnerships with government, non-government organisations, and health professionals. Foster participants' capacity to self-advocate, self-direct, and manage their lives as they choose. Maintain accurate case notes and prepare reports in line with NDIS requirements. Promote recovery-oriented, strength-based support in every interaction. About You We are looking for passionate, experienced, and person-centred professionals who meet the following criteria: Key Requirements Experience in Support Coordination, Psychosocial Recovery Coaching, Case Management, or a related role, connecting participants with diverse services. Excellent interpersonal skills that value diversity and foster strong relationships with stakeholders. Creative problem-solving skills with a strong focus on solutions and innovation. Proven ability to manage complex challenges, barriers, and risks collaboratively. Experience working within a team, demonstrating excellent communication and a commitment to continuous learning. Comprehensive knowledge of the NDIS framework, including operational guidelines and community-based support coordination. Desirable Qualifications Allied Health or other relevant tertiary qualification (highly valued but not essential). Knowledge and experience in mental health, complex health conditions, or housing support. Mandatory Requirements A genuine desire to provide high-quality support for individuals with psychosocial and other disabilities. Awareness of mental health issues and trauma-informed care principles. Australian Driver’s Licence and access to a reliable, registered, and insured vehicle. Current Working with Children Check (WWCC). Current NDIS Worker Check (NDISWC). Fully vaccinated against COVID-19 First Aid Certificate (preferred). Eligibility to work in Australia.
