Position Overview: We are seeking a passionate and dedicated Social Worker to join our team as a Care Coordinator. The role would initially be specialising in veteran services with the plan to branch out further. The ideal candidate will have experience in providing social work services in a healthcare setting, with a strong understanding of the unique needs of veterans and client's in the healthcare system. The Care Coordinator will play a critical role in ensuring our clients receive seamless and coordinated care, addressing both medical and psychosocial needs. Key Responsibilities: Conduct comprehensive assessments of veteran patients to identify their physical, psychological, and social needs. Develop, implement, and monitor individualised care plans in collaboration with on-site healthcare providers, clients, and their families. Coordinate and facilitate access to healthcare services, community resources, and support networks. Advocate for clients to ensure they receive appropriate and timely care. Provide counselling and support to veterans and their families, addressing issues such as PTSD, substance abuse, and other mental health challenges. Assist patients with navigating the health system and accessing veteran-specific benefits and services. Maintain accurate and up-to-date patient records and documentation. Participate in multidisciplinary team meetings to discuss patient care plans and progress. Conduct outreach and education activities to increase awareness of available resources and support for veterans. Stay informed about best practices and advancements in veteran care and social work.
