RN Case Manager

Job Description Job Description Join us as RN Case Manager, – Make an Impact! Location: Bronx, NY 10461 Duration: 9 Weeks (Contract) Extension Shift: 8 hrs. Days, 9:00 am 5:00 pm, (M-F) Scheduled Hours: 40 Hours a Week . Weekly Pay: $56.00 – $64.00 Job Duties: Review patient records to assess medical necessity and appropriate level of care. Coordinate care plans and discharge planning with providers and patients. Ensure services meet insurance and regulatory guidelines. Collaborate with physicians, nurses, and insurance companies. Monitor patient progress and resource use. Document utilization reviews and maintain compliance with policies. Minimum Qualification: Must have an Active RN License issued by NYSED At least two years of experience.
