Description Job Title: PT, Home Health Location: Phoenix, AZ Sh ift: Mon – Fri, 8am – 5pm, Days (5 x 8hrs) Weekly Hours: 40 Hrs. 13 weeks contract with options to extend Mileage reimbursed at 36 cents per mile for home health visits. 13 weeks M-F, 8am - 5pm, 40 hours per week No on call or weekends Coverage for East Valley Home health required HCHB desired 30 units per week Requirements BLS through AHA only 1 -2 years of PT Home Health preferred, will take someone and train them on Home Health. Active AZ PT or Compact PT License HomeCare Homebase (HCHB) EMR. Client will train on EMR! Current driver's license.
